Enrollment Fee:

Single: $325.00 for 12 months
Family: $300.00 per family member (unmarried children under 19)

Preventative Diagnostic Exams & X-Rays
Comprehensive Exam per Member (one per member)*new patients
Periodic Exams (2 per plan year) *for existing patients
Bitewings (1 set per plan year)
Prophylaxis Cleaning-Adult or Child (2 per plan year) *does not include periodontal treatment
Fluoride-Adult or Child (2 per plan year)
Complete Series or Panorex (1 every 3 plan years) 50%
All other procedures Reduced 20%

Plan year (12 month period) membership fee is due in full upon joining.
This is a discount plan, not a dental insurance plan. The plan may be enhanced from time to time.
